[ARM] 5032/1: Added cpufreq support for pxa27x CPU

PXA cpus maximum frequency depends on the cpu (624 for
pxa270, 520 for pxa272, 416 for pxa271). It should be
provided on kernel or module start (cpu-pxa
pxa27x_maxfreq parameter).

Make use of cpufreq_frequency_table_cpuinfo (patch by Bill
Reese provided by Philipp Zabel).

Some additionnal fixes from Philipp Zabel include :
 * rename PXA cpufreq driver to reflect added PXA27x support
 * remove unused variable ramstart from PXA cpufreq driver

Commit 84f43c308b73a6a12128288721a1007ba4f1a8da "pxafb: introduce register
independent LCD connection type for pxafb" implements compatibility mode
for old style pxafb_mach_info initialization data wrongly, causing the
system to Oops repeatedly - first during probe, then when drawing. Fix it
and make pxafb_decode_mach_info void.

In commit e100bb64bf7cdeae7f742a65ee1985649a7fd1b4 (mac80211:
QoS related cleanups) I accidentally changed a variable from
int to u16 causing a warning that a comparison for < 0 was always
false. John thought this was a missing deletion of code and removed
the warning by deleting the never executed branch of code in commit
3df5ee60f1ee559b1417397461891f8b483e8089 (wireless: fix warning
introduced by "mac80211: QoS related cleanups") but the problem really
was my mistake of using a u16 variable for the queue variable when
that variable can also contain an error code. This patch restores
the original code and variable type.

As reported by Filipus Klutiero <chealer@gmail.com>,
the rt2x00 Kconfig entries should be updated with
specific chipset notifications.

This cleans up Kconfig by explicitly mentioning the
supported chipsets for each drivers, and uses the
same chipset family names as mentioned on the Ralink website.

This fixes several problems I had:

* when removing the card while the card was scanning or associtating,
  it could happen that destroy_workqueue() stuck and didn't return.
* make sure the command function doesn't run while we remove the list
  of pending commands
* for still unknown reason, I had calls to lbs_stop_card() with
  priv==NULL

 * makes the queue number passed to drivers a u16
   (as it will be with skb_get_queue_mapping)
 * removes the useless queue number defines
 * splits hw->queues into hw->queues/ampdu_queues
 * removes the debugfs files for per-queue counters
 * removes some dead QoS code
 * removes the beacon queue configuration for IBSS
   so that the drivers now never get a queue number
   bigger than (hw->queues + hw->ampdu_queues - 1)
   for tx and only in the range 0..hw->queues-1 for
   conf_tx.

Use the rt2x00 queue enumeration as much as possible,
removing the usage of the mac80211 queue numbering
wherever it is possible.

This makes it easier for mac80211 to change it queue
identification scheme without having to deal with
big changes in the rt2x00 code.

rt2x00: Remove DRIVER_SUPPORT_MIXED_INTERFACES

A lot more is needed to support mixed interfaces then just this flag,
such things need new redesigns in rt2x00lib to actually make it happen.
So far there doesn't exist a Ralink chipset that can handle it, so
we might as well remove the flag for now and see what should be done
whenever a device appears that can handle it.

If the driver has set the set_rts_threshold() callback function
to mac80211 it is capable of generating RTS and CTS-to-self frames
inside the hardware and rt2x00lib doesn't have to create them
in software.
Only rt2800pci and rt2800usb support this feature.

This patch removes the IRQ-disable from mac_suspend.
The main advantage of this is to get rid of the IRQ-sync call in mac_suspend.
We need to remove the MAC suspend bit from the IRQ service mask, as otherwise
the IRQ handler would race with us.

1 - moving init_drv from handler to regular function call
2 - move the init driver flow from iwl4965 to iwlcore, thus unify it to all
iwl family as a single flow

This patch distributes the Local Oscillator calibration bursts over time,
so that calibration only happens when it's actually needed.
Currently we periodically perform a recalibration of the whole table.
The table is huge and this takes lots of time. Additionally only small bits
of the table are actually needed at a given time. So instead of maintaining
a huge table with all possible calibration values, we create dynamic calibration
settings that
a) We only calibrate when they are actually needed.
b) Are cached for some time until they expire.
So a recalibration might happen if we need a calibration setting that's not
cached, or if the active calibration setting expires.
Currently the expire timeout is set to 30 seconds. We may raise that in future.

This patch reduces overall memory consumption by nuking the
huge static calibration tables.

This patch has been tested on several 4306, 4311 and 4318 flavours.

The current code checks for the special signature that signifies
a revision 4 SPROM. Now that a rev. 8 SPROM with a 440-byte length
has been found that may not have any special code, this check could
be relaxed. With this patch, if the CRC is incorrect for a 256-byte
SPROM, the code will immediately check for a 440-byte SPROM. If there
is still a CRC error, the size is set to 440 bytes, which allows dumping
of most of any 512-byte SPROM if one is encountered.

1) the uCode doesn't know about TKIP-MMIC failure, if uCode set

3) check the keyidx of the key in the uCode before removing it upon
disable_key call

mac80211: Add IEEE80211_KEY_FLAG_PAIRWISE

This adds a new flag to the ieee80211_key_conf structure.
This flag will inform the driver the key is pairwise rather then
a shared key.

This is important for drivers who support both types of keys,
and need to be informed which type of key this is. Alternative
would be drivers checking the address argument of set_key(),
but it will be safer when mac80211 is more explicit.

mac80211: Replace ieee80211_tx_control->key_idx with ieee80211_key_conf

The hw_key_idx inside the ieee80211_key_conf structure does
not provide all the information drivers might need to perform
hardware encryption.

This is in particular true for rt2x00 who needs to know the
key algorithm and whether it is a shared or pairwise key.

By passing the ieee80211_key_conf pointer it assures us that
drivers can make full use of all information that it should know
about a particular key.

Additionally this patch updates all drivers to grab the hw_key_idx from
the ieee80211_key_conf structure.

    in ipath_cancel_sends()

        We need to unconditionally set ABORTING.  So, swap the tests
        so the set_bit() isn't shadowed by the &&.

        If we've disarmed the piobufs, then we need to unconditionally
        set DISARMED.  So, move it out from the overly protective if
        at the bottom.

    in sdma_abort_task()

        Abort_task was written knowing that the SDMA engine would always
        be reset (and restarted) on error.  A recent change broke that
        fundamental assumption by taking the restart portion and making
        it conditional on a link status change.  But, SDMA can go boom
        without a link status change in some conditions.

Now that we always use PIO for vl15 on 7220, we could get stuck forever
if we happened to run out of PIO buffers from the verbs code, because
the setup code wouldn't run; the interrupt was also ignored if SDMA was
supported.  We also have to reduce the pio update threshold if we have
fewer kernel buffers than the existing threshold.

Clean up the initialization a bit to get ordering safer and more
sensible, and use the existing ipath_chg_kernavail call to do init,
rather than doing it separately.

Drop unnecessary clearing of pio buffer on pio parity error.

Drop incorrect updating of pioavailshadow when exitting freeze mode
(software state may not match chip state if buffer has been allocated
and not yet written).

If we couldn't get a kernel buffer for a while, make sure we are
in sync with hardware, mainly to handle the exitting freeze case.

IB/ipath: Fix count of packets received by kernel

The loop in ipath_kreceive() that processes packets increments the
loop-index 'i' once too often, because the exit condition does not
depend on it, and is checked after the increment. By adding a check for
!last to the iterator in the for loop, we correct that in a way that is
not so likely to be re-broken by changes in the loop body.

IB/ipath: Fix bug that can leave sends disabled after freeze recovery

The semantics of cancel_sends changed, but the code using it was missed.
Don't leave sends and pioavail updates disabled, and add a comment as to
why the force update is needed.

If a send work request has immediate errors and is not put on the
send queue, we shouldn't update any of the QP state.

The increment of the SSN wasn't obeying this.

We warn about prototype chips, but the function that checks for
support is also called as a result of a get_portinfo request, which
can clutter the logs.

Restrict warning to only appear during initialization.

[CRYPTO] hmac: Avoid calling virt_to_page on key

When HMAC gets a key longer than the block size of the hash, it needs
to feed it as input to the hash to reduce it to a fixed length.  As
it is HMAC converts the key to a scatter and gather list.  However,
this doesn't work on certain platforms if the key is not allocated
via kmalloc.  For example, the keys from tcrypt are stored in the
rodata section and this causes it to fail with HMAC on x86-64.

This patch fixes this by copying the key to memory obtained via
kmalloc before hashing it.

get_part() is fairly expensive, as it O(N) loops over partitions
to find the right one. In lots of normal IO paths we end up looking
up the partition twice, to make matters even worse. Change the
stat add code to accept a passed in partition instead.

We currently set all processes to the best-effort scheduling class,
regardless of what CPU scheduling class they belong to. Improve that
so that we correctly track idle and rt scheduling classes as well.
